Fill Out A Form

Hello,

My name is Krisna Jiwani Dewi Dasi, absent number 22 from X MIPA 1 class.

Last Friday, my english teacher gave my class an assignment to watch a video. She gave us three questions about the video that we must answer and write the answers here.

The questions are:
1. What is it about?
2. Do you understand it?
3. Can you summary it?

And my answers for those questions are:
1. It’s about form and how to fill it out.
2. Yes, I do.
3. Yes, I can.
    The summary of the video,

       There are so many kinds of form. For example are library form, job application form, registration form, bank form, and questionnaires. Functions of the form are to record a personal enquiry, to apply for something, to makes a booking or reservation, and to ask people for their opinion. To fill out a form, there are a number of questions that we must answer. The questions are, “What’s your name?” “When were you born?” “Where were you born?” “Where do you live?” and much more. 

I have made a short story about filling out a form. 

Mr. Smith : “Good morning Mr. Brown.” 

Mr. Brown : “Good morning, what’s happen with your arm Mr. Smith?” 

Mr. Smith : “I broke my arm when I was exercising yesterday.” 

Mr. Brown : “Ouh, that’s must be so hurt. Just tell me if you need some help.” 

Mr. Smith : “Thank you Mr. Brown. Coincidentally, I need your help now. I have to fill out a form, can you help me to fill it out?” 

Mr. Brown : “Of course, where is it?” 

Mr. Smith : “Here it is.” 

Mr. Brown : “Okay first, may I know your full name?”

Mr. Smith : “My name is Samuel Smith.” 

Mr. Brown : “What a good name. Next, where do you live?” 

Mr. Smith : “I live in Ave Rowena, Los Angeles.” 

Mr. Brown : “How old are you?” 

Mr. Smith : “I’m 40 years old.” 

Mr. Brown : “Where were you born?” 

Mr. Smith : “I was born in Bali.” 

Mr. Brown : “Wow, I love Bali. Next, when were you born?” 

Mr. Smith : “I was born on July 27th 1980.” 

Mr. Brown : “Last, may I know your e-Mail address?"

Mr. Smith : “My e-Mail is smithsam@gmail.com.” 

Mr. Brown : “Finish! Here it is.” 

Mr. Smith : “Thank you so much Mr. Brown.” 

Mr. Brown : “You’re welcome.”
